Aultman, Mercy Medical, and North Canton Medical Foundation are participating medical facilities. Pick up a card at any of the above, have a doctor or clinician sign it and state the dates you will be in therapy. Then simply call, make a tee time and come out to enjoy a round of golf in the fresh air.
